Overview
Healthcare in inZOI revolves around treating illnesses at home using First Aid Kits. There are no functional hospitals in the current Early Access version. Zois can get sick from cold weather, contagion from other Zois, pollen allergies, and food poisoning. Untreated illness can worsen over time and, in severe cases, lead to death. Prevention is handled through the Vaccine Donut, an item from the Meow Store that grants permanent immunity to colds and allergies.
Illness Types
Zois can contract several types of illness, each with different causes:

How Zois Get Sick
Weather
Zois are more likely to catch colds and flu during colder weather. Winter months significantly increase sickness rates across the city. Keeping Zois warm and indoors during cold snaps helps reduce the risk.
Contagion
Sickness is highly contagious within households. If one member of a family gets sick, it will not be long before the entire household is coughing and sneezing. Living in close quarters with a sick Zoi spreads the illness quickly, making prompt treatment essential.
Food Poisoning
Two food-related mechanics can cause food poisoning:
Spoiled food: Leaving cooked or raw food out for too long causes it to spoil. In the current version, spoiled food cannot be eaten; Zois can only smell it or throw it away. However, spoiled food in refrigerators or improperly stored ingredients can still cause issues.
Low Cooking skill: Zois with low Cooking skill are more likely to prepare meals that cause illness. Low Cooking skill also increases the risk of kitchen fires, which are a separate hazard entirely.
Allergies
Pollen triggers seasonal allergies in some Zois, causing them to feel Uncomfortable with itchy nose symptoms. Allergies can last several in-game days if untreated.
Symptoms and Effects
Sick Zois display several visible symptoms:
Coughing and sneezing: "Cough! Cough!" text appears above the Zoi's head. Other Zois nearby react with disgust.
Uncomfortable emotion: Sickness triggers the Uncomfortable emotional state, lowering overall mood
Performance penalties: Sick Zois perform worse at work and school. Prolonged illness affects career progression and grades.
Social impact: Coughing in public causes other Zois to look at the sick Zoi with disgust, which can strain relationships
Treatment
First Aid Kit
The primary treatment method is the First Aid Kit, available in Build Mode for 100 Meow. Place it anywhere in your home. To use it:
Select your sick Zoi
Click on the First Aid Kit
Choose "Take Cold Medicine"
The Zoi sparkles and lights up, indicating the illness is cured
Each First Aid Kit provides unlimited cold medicine. You never need to refill or replace it. All premade houses and apartments come with at least one First Aid Kit already placed.
Vaccine Donut
The Vaccine Donut is a preventive item available from the Meow Store. Feeding it to a Zoi grants permanent immunity to colds and allergies. This is the only way to prevent illness rather than just treating symptoms. The donut is a one-time purchase per Zoi and eliminates the need for repeated First Aid Kit visits.
Kitchen Fires
While not a healthcare system per se, kitchen fires are closely related to food preparation risks. Fires can start when:
A Zoi with low Cooking skill attempts to cook a meal
Broken appliances are left unattended
Food is left on a stove or in an oven too long
Kitchen fires can cause death by fire, one of 17 death types in the game. Leveling Cooking skill reduces fire risk significantly.
Hospitals
Hospital buildings are visible on the city map (the Dowon Medical Station is referenced in illness notification messages), but they are not functional in the current Early Access version. Zois cannot visit a hospital for treatment, check-ups, or medical procedures. The developers have stated that implementing functional hospitals would be "too complex" for the current phase of development. All illness treatment must be handled at home using First Aid Kits.
Hospitals may be added in future updates, and the community has also expressed interest in a medical career path where Zois could attend medical school and work as doctors or nurses. Neither feature has been confirmed with a timeline.
Death from Disease
Disease is one of the 17 confirmed death types in inZOI. If a Zoi's illness goes untreated for an extended period and progresses to an infection, the Zoi can die. This applies to Zois from the Teenager life stage onward. Children cannot die from illness; if both parents die, children are sent to the Child Protection Agency.
Upon death from disease, the Zoi may become a ghost with food-poisoning-type abilities (making nearby living Zois feel sick, breaking toilets and refrigerators) if their Karma is insufficient for a peaceful afterlife.
Tips
Buy a First Aid Kit (100 Meow) immediately when starting a new household. It is the only way to treat illness.
Treat sick Zois quickly. Sickness spreads to other household members within hours.
The Vaccine Donut is the best long-term investment. One purchase per Zoi prevents all colds and allergies permanently.
Level Cooking skill early. Low skill means higher fire risk and a greater chance of preparing food that causes illness.
Do not leave cooked food sitting out. Store it in the refrigerator or eat it promptly to avoid spoilage.
During winter, keep Zois warm and indoors when possible to reduce cold/flu risk.
Lower the city sanitation setting if you want a more challenging illness experience, or raise it for a healthier city.
