The Plague Witch fights as a backline caster rather than a melee threat. Her two threats are ranged plague hexes and the Plague Thrall minions she summons to screen herself. On her own she cannot pressure a prepared player, so the fight is about closing distance and cutting the cast short.

Because her Defence is high but her health pool is small, the most efficient kill is a focused Pierce weapon such as a rapier or Muskets build. Carrying a Pain Relief Elixir lets you ignore the hex chip damage and commit to the rush.
Plague Witches are human (or formerly human) spellcasters who pacted with the same supernatural forces driving the Cursed Swamps' corruption. Whether they served the High Priestess directly, made independent pacts with the same dark powers Blackbeard used, or were once Brethren of the Coast magicians who fell to corruption, is part of the swamp's unfolding mystery. Their role as both combatants and Plague Thrall summoners suggests they are middle-tier servants of whatever older intelligence is orchestrating the broader plague.

Plague Witches are the main reason to clear Cursed Swamps ruins on a Quagmire Powder run. Each kill drops one to two units (1 at 75 percent, 2 at 25 percent), and the powder is a hard requirement for Mire Metal Ingot smelting, so witches gate your endgame smithing. They share the swamp with Plague Thrall packs and the High Priestess, whose domain they guard as acolytes. Clearing the acolytes first thins the fight before you reach the boss.
Hotfix 0.10.0.3.104 on April 19, 2026 fixed a feather simulation bug on the Plague Witch model. Before the patch, the hat feathers would occasionally detach or animate incorrectly during teleport and hex-cast animations, producing visible clipping artefacts during the fight. The fix is cosmetic; no combat stats, damage values, or drop-table numbers changed with this patch.
