Overview
Pregnancy in inZOI lasts 3 in-game days and progresses through three visible stages. It requires a specific action separate from intimacy, is not guaranteed on the first attempt, and produces a newborn that ages into a Toddler after 1 day. The system has received significant updates since launch. Earlier versions required marriage, but v0.2.0 (June 2025) removed that restriction and added two alternative methods for having children: adoption and Medical Birth.
There are now three ways to add a child to a household: biological pregnancy, adoption, and Medical Birth. Only female Zois can become pregnant in the current Early Access build.
Requirements
For a biological pregnancy attempt to be possible, several conditions must be met:

The couple must have a strong romantic relationship. Casual acquaintances cannot attempt pregnancy.
The household must have fewer than 8 members. At maximum family capacity, pregnancies automatically fail.
Only female Zois can become pregnant.
Both Zois must be at the Young Adult Adult life stage or older (Young Adults and above).
Marriage is not required. This restriction was removed in v0.2.0 (June 2025). Unmarried couples with a strong enough romantic bond can now have children.
Initiating Pregnancy
Pregnancy is initiated through a dedicated action, not through the general intimacy interaction. To start:
Click on the partner Zoi.
Select More, then navigate to the Romance category.
Choose "Suggest Having a Baby."
The couple typically moves to a bedroom.
Success is not guaranteed. The game rolls a probability check, and failure means the Zoi simply did not conceive. Multiple attempts may be necessary. There is no visible probability number or fertility stat.
Confirming Pregnancy
When conception succeeds, a red exclamation mark appears on the female Zoi's portrait icon. Click it to prompt the Zoi to take a pregnancy test. She will walk to a toilet, use it, and a notification will confirm or deny the pregnancy. Until the test is taken, pregnancy is not officially confirmed in the game's records.
Pregnancy Stages
The 3-day pregnancy divides into three distinct stages, each lasting approximately one in-game day (roughly 96 real-world minutes at default speed):

Throughout all three stages, appearance editing is disabled. The player cannot change the Zoi's clothing, hair, or body through the character creation editor until after the birth.
Special Pregnancy Interactions

Morning Sickness | The pregnant Zoi may feel nauseous and can use the toilet. This triggers the Uncomfortable emotion, which can be alleviated by taking cold medicine, eating food, sleeping, or taking a bath. |
Giving Birth
After 3 in-game days, a Give Birth prompt appears as a red exclamation mark over the Zoi's head. Clicking it causes the Zoi to automatically travel to the hospital. A loading screen plays, followed by a brief cutscene showing the Zoi in a hospital bed holding the newborn. The player is then prompted to name the baby and assign a gender.
If the Give Birth prompt is missed or dismissed, the birth still happens automatically after approximately 5 in-game hours. The baby will appear in the household crib with a random first name and the father's last name. The name can be changed later through the Zoi's profile.
Baby gender is random and cannot be influenced in the current build. Players have requested gender selection or influence mechanics on the official forums, but these remain wishlist items.
The Newborn
The newborn appears in a crib in the household. A trait is randomly assigned at birth. Physical features like hair color, eye color, and facial structure are inherited from both parents through a genetics system that blends traits from both parents. The v0.2.5 hotfix specifically fixed an issue where "facial features of newborn Zois generated via genetics appeared unnatural."
Newborn Care (Day 1)
The Newborn life stage lasts exactly 1 in-game day, after which the baby automatically ages into a Toddler. During the Newborn stage, interactions are very limited:
Check Their Temperature (the only Parenting menu option)
Talk to the baby and show affection (builds relationship)
Socialize and observe the baby
Newborns cannot be fed (feeding only unlocks at the Toddler stage), cannot be held or carried around, and are confined to their crib at all times.
Toddler Care (Days 2 to 3)
Once the baby ages into a Toddler, the range of available interactions expands significantly:

If a toddler's needs (particularly hunger) are severely neglected, the Child Protection Agency will remove the child from the household rather than the child dying.
Babysitter Service
Parents can hire a babysitter through the Pocketmarket phone app. Two service tiers are available:

Babysitters handle feeding and diaper changes. The service is only available for households with toddlers, not newborns.
Life Stage Durations
For reference, here are the durations of each life stage. One in-game day equals roughly 48 real-world minutes at the current default speed (changed from 96 minutes in v0.4.0).

The ageUp cheat can force instant aging. Day length can be adjusted in settings, which affects real-world time for each stage.
Twins
Twins are not currently available in the game. Early Access promotional materials showed families with two babies, hinting at twins, but no player has confirmed getting twins in regular gameplay despite extensive testing. No cheat code exists for forcing multiple births.
The official 2026 roadmap explicitly lists "Implement multiple pregnancies such as twins and triplets" as a planned feature under the "Fundamentals First" initiative, but no specific release date has been given.
Adoption
Adoption was added in v0.2.0 (June 2025) and provides an alternative path to expanding a family without biological pregnancy. This allows same-sex couples, single Zois, and anyone meeting the requirements to add children.
To adopt:
Go to a computer or laptop in your home.
Select "Adopt a Child" under General Actions.
Choose the child's age: Newborn, Toddler, or Child.
Confirm the adoption.

Medical Birth
Medical Birth ("Create a Child with Medical Help") was also added in v0.2.0 and provides a third method for having children. It costs 1,000 Meow and works for all gender combinations, including same-sex couples.
To use Medical Birth:
Go to a computer or laptop.
Select "Create a Child with Medical Help" under General Actions.
Choose to go solo (child inherits only your traits) or with a partner (child inherits both parents' traits).
If choosing a partner, the other Zoi must give consent.
Known bug: For unmarried couples (especially same-sex), the baby may only recognize the Zoi who initiated the Medical Birth as family. The partner is not recognized as a parent. This has been reported on the official forums.

Community Discoveries
Players have discovered several notable quirks and exploits related to pregnancy and family:
NPC autonomous pregnancy: Married NPC Zois can get pregnant autonomously without player intervention. A "No NPC Babies" mod exists on CurseForge to address this.
Pregnancy getting stuck: A widespread Early Access bug where Zois remain pregnant indefinitely. The workaround is using the removePregnancy cheat followed by helloMyAngel.
Pregnancy bump persisting: Some Zois retain their pregnancy bump after giving birth or after using removePregnancy.
