How to Increase Defense in Crimson Desert is a multi-layered problem that combines four armor slots, three armor classes, refinement levels, reinforcement, Abyss Gear, and buff food. Squeezing every layer pushes the total damage you can take per hit noticeably higher.

The Four Armor Slots
Equip all four slots for full defense. Leaving a slot empty forfeits a meaningful chunk of your total DEF total.

Refinement
Take every armor piece to the maximum refinement level at a smithy (up to level 10). See the Weapon Refinement guide for materials; the same process applies to armor.
Reinforcement
Reinforcement is a separate, free temporary defense boost applied at the anvil. It resets on certain events, so max it before every major fight.
Abyss Fortification Gears
Socket Fortification I, II, and III Abyss Cores into every armor piece for an additive defense boost:

Accessories and Food
Accessories (rings, earrings, necklaces) can also be refined for additional stats.
Elemental Resistance foods grant temporary defense against specific damage types and are essential for elemental bosses.

Refine every piece to level 10.
Max Reinforcement at the anvil before every major fight.
Socket Fortification Abyss Gears in every slot, I to III as they unlock.
