To attempt pregnancy, couples must be a married male-female pair living together with a strong romantic relationship and fewer than 8 household members. Only female Zois can become pregnant in Early Access. Adoption and same-sex biological children are not yet available.
Initiate a social interaction with a spouse, navigate to Romance dialogue options, and select "Suggest Having a Baby." The couple typically heads to a bedroom. Success is not guaranteed -- multiple attempts may be necessary.
A red exclamation mark appears on the female Zoi's portrait. Click it to prompt a pregnancy test -- the Zoi uses a toilet to perform the test, and results are announced via notification.
Pregnancy lasts 3 in-game days with visible belly growth. Pregnant Zois gain special interactions like announcing the pregnancy and experiencing food cravings. Appearance editing is disabled during this time.
After three days, a "Give Birth" prompt appears. The Zoi either gives birth on-site or travels to the hospital. Missing the prompt means losing the cutscene and the chance to name the baby immediately.
The newborn appears in a crib and receives a randomly assigned trait. The Newborn life stage lasts 1 in-game day before automatically aging into a Toddler. Parents can interact with newborns, and basic needs are handled automatically.
Early Access teasers have shown families with two babies at once, suggesting twins may be possible.
