Esme is the mother of Coen and the wife of Pieter. She lives with the family in the hills of Laslea, the secluded village where Coen was born and raised.

Esme has always put her family first, often at the expense of her own well-being. That habit of self-neglect is the defining thing the developers have said about her, and it frames how the household works: the burden she will not set down is one of the reasons Coen, as the eldest child, took on so much responsibility so early.
Declining Health
Since Brencis rose to power in Vale Sangora, Esme's condition has worsened drastically. She no longer speaks, eats, or sleeps properly, and her health continues to decline while the family struggles to hold itself together. Official material describes Coen from the outset as a young man with a sick mother and a family to take care of, so her illness is part of the starting situation rather than something that develops during the story.

The Black Plague swept through Laslea and brought heavy losses to its inhabitants, and vrakhiri blood is known to have healing properties for humans, but neither thread has been tied to Esme specifically.

Role in the Story
The vrakhiri capture Coen's family, and recovering them is the reason he has to learn to use his new powers at all. The player has 30 days and 30 nights to save them, which is the clock that governs the whole game. See the 30-Day Cycle for how that deadline works in practice. The family is held at Brencis's castle to be sacrificed at the ceremony marking his investiture as Prince, which is what sets the month-long limit.
In the Prologue
Esme is on screen within minutes of the game starting. The prologue quest Withering Away opens with the household trying to get her to eat, and the first real decision the player is offered is whether Coen should take the spoon and feed her himself. Both answers end the same way, with the spoon knocked aside and Esme in tears, and the choice carries no long-term consequence. It is placed there to introduce the idea that the game will ask, not to branch anything.
What follows does branch. Pieter sends Coen to Anca on the edge of the village for herbs, and Anca hands over a written recipe for a calming medicine so that Esme can get through the evening's blood mass. Back at the house the game will not let the player re-read the page while brewing, so the instructions have to have been remembered: hot water, then three spoonfuls of herbs. Brewing it correctly is not optional in any meaningful sense. Get it wrong and Esme dies during the prologue, and her absence carries all the way through to the closing sequence, where the tableau covering Coen's family shows the household without her.
In the Nightmare
Esme also appears outside the main story. A quest triggered at a tower in the south of the valley traps Coen in a repeating dream of his own home, in which he eats breakfast with his family, watches them carried off by a flying creature, and fights through spectres to get them back. Escaping it requires reading a funeral scene in the village church correctly, and Esme is one of the three figures there who has to be examined in order. A player who fails to take the hint and runs the loop four times is spoken to by Esme on the fifth morning, who tells Coen he is staying, and the game ends there. It is the only documented ending that closes without the post-credit scene.

The shipped game answers the question pre-release material left open about whether Esme survives the opening: that depends on the player, since a wrongly brewed medicine kills her in the prologue and her absence then carries through to the epilogue.
