The Tomb of Perdition becomes accessible after completing the Dry Valley Abyss and unlocking Tommaso in the Tashkalp region. Once those prerequisites are met, the puzzle entrance appears within the Abyss area accessible from the Dry Valley. The core mechanic of this puzzle involves a rotating cage-like structure with multiple openings. Each opening contains either an axiom piece or a socket where a piece must be placed. You need to collect three axiom pieces (two rectangular and one square) and insert them into their matching sockets by rotating the cage to expose different openings. The Axiom Force ability is used to pick up and carry the pieces, while Force Palm locks them into place.
Approach the Abyss Nexus at the entrance of the Tomb of Perdition and activate it. This serves as your checkpoint and establishes a fast travel point for this puzzle area.
Head to the right-side opening of the rotating cage. Inside, you will find two rectangular axiom pieces. Use Axiom Force (L3) to pick them up and bring both of them out of the cage opening.
Use Axiom Force on the yellow dial at the center of the mechanism. The dial will turn blue, confirming activation. This rotates the golden cubes within the cage structure and shifts the accessible openings, making the left-side opening accessible.
Move to the left-side opening that is now accessible. Inside you will find the square axiom piece. Pick it up with Axiom Force. While you are at this opening, place one of the rectangular axiom pieces into the socket you will find here. Use Force Palm (R3) to slot it into place and lock it securely.
Return to the central mechanism and activate the dial again. This second rotation shifts the cage openings once more, bringing the right side back into an accessible position with a new socket revealed.
Go back to the right-side opening. Insert the square axiom piece into its designated socket. Use Force Palm to lock it into place. Make sure it seats properly before moving on.
Pick up the remaining rectangular axiom piece and climb onto the rotating cage structure itself. Locate the final socket on the opposite side of the structure. This socket is positioned higher up on the cage, which is why you need to climb. Place the last rectangular piece into the socket and secure it with Force Palm.
With all three axiom pieces locked into their sockets, the mechanism responds. Use Force Palm on the Abyss Core to trigger the final activation sequence. The puzzle is now complete.
After completing the main puzzle, do not leave immediately. There is a hidden chest that is easy to miss if you rush toward the exit. Climb onto the rotating cage structure (the mechanism you have been rotating throughout the puzzle). Stand facing the fast travel point or entrance direction. Look along the right side wall of the cage for the hidden chest. It contains additional rewards that complement the main puzzle completion loot.

Keep track of your axiom pieces. You are managing three pieces across multiple rotations. Set pieces down in clear, memorable spots so you do not lose track of them between rotations.
Force Palm every socket. Simply placing a piece near a socket is not enough. You must use Force Palm (R3) to lock each piece into place. If a piece is not properly secured, the puzzle will not register it as complete.
Check both sides after every rotation. Each rotation changes which openings are accessible. Explore both sides after rotating to ensure you are not missing a socket or piece.
Do not skip the hidden chest. The rewards from the hidden chest on the right side of the cage are worth the extra effort. Climb the cage after completing the puzzle to collect them.
The Surge Abyss Gear is worth the effort. This gear piece significantly improves your combat performance in later Abyss encounters, so completing this puzzle early in your Abyss progression is recommended.
