Itzeltek Sentinel is a hostile creature in Windrose, a large guardian crocodile that prowls the wetland edges of the Cursed Swamps. It is not a wandering ambient spawn like the common reptiles of the marsh: the Sentinel is a scripted miniboss tied to the Jokes of the Gods side quest, where it ambushes the player at one of the swamp's sacrificial altars. The drop table marks it as a mid-game opponent dangerous to underleveled crews but a reliable source of crocodile materials and Plague Wood for those who can bring it down.
The Itzeltek Sentinel appears during Jokes of the Gods, a hidden quest begun at a sacrificial altar after you enter the Cursed Swamps. The quest sends you between altars to piece together the legend of a priest of the dead, and the Sentinel attacks the moment you copy the final inscription into your journal. In appearance and moveset it is a much stronger version of the Plague Crocodile that roams the same biome. It is a level 15 enemy, so a crew that has only just reached the swamp should expect a hard fight rather than a quick kill.

Spawns in the Cursed Swamps biome, usually near stagnant pools and overgrown ruins, and is guaranteed to appear at the final altar of the Jokes of the Gods quest. The exact altar and spawn points are seeded by world generation, so a single map's location list does not transfer to another player's world.
The Sentinel fights like an oversized crocodile: heavy lunging bites and a tail sweep, each telegraphed before it swings. The reliable approach is to break away from the altar into open ground, then read each wind-up and dodge to the side or back depending on the attack. Because it has no ranged option, you can open distance to heal and chip it down with a Blunderbuss, Musket, or Pistol between its rushes. Fighting in tight reeds, where the swamp slows your movement, is the main way crews lose this encounter.
Pull the Sentinel onto open ground rather than fighting in tall reeds. Plague Wood and Tainted Bile drops are easier to spot when the corpse is not lost in vegetation.
Bring a stack of Healing Herbs and a way to clear poison. Swamp creatures often apply a poison status that drains health over time, and waiting it out is rarely worth the lost combat efficiency.
Outrun the Sentinel when your health is low. It cannot hit you at range, so creating space to heal and reposition is safer than trading blows toe to toe.
