Inside the side room, locate an unlit lantern.
Light the lantern. Through a hole in the wall, you can see the gate slide open.
Return to the chest and collect the Red Needle.

The Red Needle is tucked deep inside a sanctum connected to Mani's Laboratory and Gate, located on the far eastern side of the world map. The trip is long, so the practical approach is to unlock a nearby teleport point first and return later rather than trying to run across the map in one go.
Fast travel to the eastern checkpoint nearest Mani's Laboratory and Gate.
Ride up the hill toward the sanctum, keeping to the ridges to avoid the swamp, which can slow or injure mounts.
Enter the sanctum and go down the first staircase. Mechanical patrols will follow you into the stairwell, where the narrow space makes them easy to deal with.
Back at the top, turn left inside the main hall, then take the immediate right and head downward.
Clear the cobwebs as you pass (they drop spider webs as a bonus material) and locate the power switch that unlocks the inner gate.
Activate the switch, backtrack up the stairs, take a right, and descend again to reach the chest containing the Red Needle.
The Red Needle comes with sockets already populated for Critical Rate, Attack Speed, and Attack. Every socket can be unslotted and transferred to another weapon, so players who prefer a different loadout still benefit from the detour. The weapon itself can also be upgraded normally for those who want to main it.
Keep a healing item on your mount before the trip; the nearby swamp can injure horses mid-ride.
The sanctum's flying patrols are aggressive but not lethal. Sprint past them if you are underleveled.

The Red Needle is stored inside a sanctum that does not need to be cleared. Enter through the front gates and look for a candle near the first corridor. Lighting the candle opens the path forward. Loop back around through the newly accessible passage, enter the chamber at the end, and grab the katana out of the chest. You can do the entire run without engaging any of the sanctum residents.
