The medical layer is one of the least documented systems in Zeverland. A handful of items and states are confirmed from the second pre-alpha tech test, the studio has said it will rethink the system, and almost nothing else has been published. No healing value, crafting recipe, dosage or acquisition source exists for any medical item.

Those four come from second-test player feedback the studio published on 2026-06-28. The build was wiped when the test closed on 2026-07-02, so they are a record of what existed in one pre-alpha, not a shipping item list. See Second Pre-Alpha Tech Test.
Players in the second test said the medical system felt disjointed: items and conditions that did not add up to a coherent whole. The studio agreed with them and said on 2026-06-28 that it would rethink the system. No revised design has been published since, which makes the medical layer one of the systems most likely to look different in the next build. Anyone reading the item list above should treat it as provisional for that reason.
The better-documented half of the medical layer is the gear that lets you survive contaminated ground. Entering a tier-one infected zone, the red fog area with low visibility even in daylight, requires protection to be worn rather than just courage.

Twenty minutes proved to be a problem rather than a budget. The second test's first main quest sat beyond an infected zone, and players died trying to reach it before their protection ran out. The studio's announced answer is a faction-based lifeline that extends the time a player can spend inside a zone, announced on 2026-06-28 and not shown since. See Infected Zones and Factions.
At gamescom 2026 the studio described the survival layer as three stats, health, water and food, and infection, with transformation triggering when infection reaches 100 percent. That account comes from a booth demo rather than from anything the studio has published itself, and no first-party post names an infection meter. One stylistic constraint bounds the whole system. The studio has confirmed there are no gore or organ effects and that the art direction is deliberately absurd and cartoony, so injury and illness are represented through items and states rather than through damage modelling on the body.
