The Medical Handbook is an antique medical reference recovered from a deceased ship's surgeon. The endpaper has been cut out, the implication is that something else was hidden inside it before the player found the book. It is a lore-flavoured Miscellaneous item rather than a usable medicine: it carries no healing effect and exists to flesh out the story of the wreck or camp it is found in.
The old handbook is full of marginal notes and comments. The back endpaper is cut off, something was obviously kept in it.

The handbook is a fixed-flavour pickup rather than a farmable drop. It turns up where a ship's surgeon would have worked or died: looted from chests and shelves at wrecks, abandoned camps, and ruined cabins around the archipelago. Because it is tied to specific spots rather than a loot pool, the exact location varies by world and patch, so treat the spawn as a one-off discovery rather than something to grind for.
The cut endpaper hint suggests this book was used for smuggling small items (a folded letter, gem, or coin) inside the binding. As a quest pickup, the handbook may itself be the actual smuggle artifact, or it may serve as the marker pointing the player to where the original contents went.
Its previous owner, the ship's surgeon, places it alongside the game's wider medical material. Where the handbook is pure flavour, items such as Medicine and the Crate of Medicine are the functional side of shipboard healthcare, and Doctor Galen is the in-world figure most associated with treating the sick. For a player's own recovery the relevant items are consumables like Healing Herbs rather than this antique reference.
