Initial version: the confirmed achievement count, the progressive trophy At the End of All Memory and the six sub-trophies it tracks, and what remains unpublished
The Relic: First Guardian carries 37 achievements. The figure comes from the PC store listing and has not moved since launch. The names and unlock conditions have almost all stayed private: seven of the thirty-seven are public, and every one of those came out of a patch note rather than a published list.
The three platforms use different words for the same idea. Patch notes call them trophies, which is the PlayStation term, while the Xbox listing advertises Xbox achievements. Whether all three versions ship an identical list has not been stated, and the Xbox and Nintendo Switch 2 versions have not released yet. See Platforms and Release for the release schedule.

One trophy has been described in detail, because it broke. At the End of All Memory is a progressive trophy: rather than unlocking from a single act, it tracks the combined progress of six other trophies. Patch 13 confirmed that its progress counter was not incrementing correctly and said a fix was already written and in internal testing. Patch 14 shipped that fix and made the counter update retroactively, so starting or loading a save after the patch corrects a total that had been counting short. The six trophies it draws on are the only other names in the public record.

The six names read like act titles, and it is tempting to map them onto the chapter structure. Nothing supports that. They have only ever been published as trophy names, and the developers have never tied any of them to a chapter, an act or a specific boss. Until one of them appears in a source that does, this wiki treats them as trophies and nothing more; the confirmed chapter structure is on the Story and Premise page.
Thirty of the thirty-seven have never been named. No source publishes the full list, the unlock conditions, the point values or the platform trophy grades, and there is no in-game completion tracker beyond the Status Board, which counts defeated bosses rather than achievements. Anyone chasing a full clear is working without a checklist.
The pattern so far is that names surface only when something breaks and a patch note has to identify what it fixed. That is how all seven current names became public, and it is the most likely route for the rest. The boss counter on the Status Board is covered on the Bosses page.
