The Chillfallen Sword lies next to a skeleton in an area that is otherwise used for a scripted boss or side encounter. Whether you are early in the game and have not yet triggered the fight, or late game having already defeated the boss, the sword stays in the same spot. If you missed it on your first pass, you can come back at any time and grab it; it does not despawn after the associated encounter finishes.
