The Broken Heirloom Medallion is received during the opening main quest How My Shore Adventure Began, the first-hour sequence that begins after the character is shipwrecked by Blackbeard and washes up on the starting island. The keepsake is the father's medallion referenced in the quest's framing.
This medallion closely resembles the Broken Ancient Medallion, a separate item recovered later from the Skeleton of Juan de la Cosa during the side quest Mystery of the Expedition. On completing that quest the journal links the ancient medallion to the one the father gave the player, alongside a reference to a faction the captain calls the Order. The two pieces are distinct inventory items, but the writing positions them as part of the same unfolding mystery. The plain heirloom counterpart is the Heirloom Medallion.
