The Blessing of the Immortal is earned at the Spire of Ringing Truth in Pailune. Many players will have already been to the spire because it is also the approach for the abyss area tied to the frost and fire unlock, so the route should be familiar.
The method is specific: enter the tower, climb to the top, then drop the bell down the shaft. Dropping the bell reveals a hidden cellar below the spire. You then drop into that basement yourself and fight the boss waiting there. Defeating that boss drops the Blessing of the Immortal key item.
When consumed, the Blessing of the Immortal grants 10 seconds of full invincibility. The Greymane takes no damage at all during that window. The Rage Gaming guide is candid that this is a niche item: 10 seconds is a short fuse and most encounters do not demand that kind of emergency button.
The clearest realistic use case is against Overwhelming Beings, the three end-tier bosses called out as the hardest fights in the game. Even there, many players report carrying the item without ever consuming it.
