The story of Stronghold 4 is told through a cinematic, fully voice-acted campaign. It is framed as a prequel, set several years before the events of the original Stronghold, and presented as the origin of that earlier tale.

The campaign follows Penryn of Wethel, a humble shepherd whose life is upended and who is forced, through trial by fire, to become a leader and commander. Along the way he must contend with hostile outlaws, rival lords, and the harsh elements of the medieval world.

The campaign runs across twenty-two story missions and sits alongside the game's other modes. It is brought to life by a voice cast that includes Ben Starr, Samantha Béart, Walles Hamonde, and Harry McEntire, with an original score by Robert L. Euvino. Beyond Penryn himself, Firefly has not yet revealed the names of other characters or factions in the story. The campaign ships with full voice acting, with full audio available in English, German, and Polish among the game's supported languages. The demo does not include story missions; the campaign's narrative is held back for the early access launch.
Firefly has singled out the presentation as new for the series: the campaign carries full cinematic cutscenes for the first time in a Stronghold game.
The prequel framing was a deliberate choice rather than a continuation. Firefly has said the campaign does not pick up the story of the boy and the wolf from the original game, because the earlier timeframe would leave that character too young to carry the lead role. Setting the story before the original also let the studio add context to characters players already know while leaving room to introduce new ones.
Firefly has said the campaign sets Penryn alongside a cast of new and returning characters, describing them as original figures placed next to familiar faces from the series. Cinematic stills released with the game show several of them, among them an eye-patched nobleman in a candlelit hall, but none has been named.
One crossover has been ruled out. Firefly has confirmed that no characters from Stronghold 2 appear in the main story, since that game's cast never crossed paths with the original's.
The supporting voice work reaches beyond the principal cast. Firefly has said the troops and townsfolk have newly written and cast voice lines, kept in the series' broad comic register and including revivals of classic lines from earlier games.
