The Otherworld is named by Konami and Screen Burn as appearing in SILENT HILL: Townfall. Producer Motoi Okamoto has referred to "the town's atmosphere and the portrayal of the Otherworld" as part of what players will find here, and a first-party post describing the June trailer places a creature "stalking Simon in the darkness of the Otherworld."
Only the name is carried over. Nothing about how the Otherworld worked or looked in earlier games has been confirmed for Townfall, and this page records only what this game's own material shows.

One Town, Two States
The clearest thing Townfall has shown about its Otherworld is a single location presented twice. The town square of St. Amelia, memorial and all, appears both under the town's ordinary heavy fog and in a red-lit version of the same space: same buildings, same monument, different light.

What sits behind that has not been published. Nothing states what triggers the change, whether Simon crosses over or the town does, or whether the player has any say in when it happens. What the two views establish is narrower and firmer: both versions of the square occupy the same ground.
How It Looks
Red is the dominant colour. Environments in the Otherworld have been described as bathed in oppressive red light, in contrast to the fog of the ordinary town, and ray tracing is used on both. The fullest first-party description came on September 16, 2026, in a post co-written by the producer and the director: the sudden, shocking change from the blue-grey fog to the bright red Otherworld, layered in pipes, alarm lights, broken buildings and stained walls. Okamoto used the same image on stage at Tokyo Game Show three days later, a red-stained world strung with countless pipes, built to press unease and claustrophobia on the player. The red and black pairing runs through the game's whole palette, and McKellan has said the two colours carry a symbolic significance for Simon that the story eventually explains. Creatures
The ranged creature that fires tentacles from its chest was encountered during the section where the Otherworld invades the streets. Like every other creature in the game, it has no published name. Descriptions are on Combat and Stealth.

A second figure has been shown inside the Otherworld: pale, naked and hairless, standing in a red-lit interior beside a wheeled trolley. It is unnamed too, and nothing about what it does has been published.
Scope
Townfall is a self-contained story set on a Scottish island in 1996. It is not set in the town of Silent Hill, and no established figure, faction or creature roster from a previous game in the series has been confirmed to appear. Anything about the Otherworld beyond what is listed above stays out of this article unless official material confirms it, which also keeps the page free of story spoilers now that the game is out.
